Heart failure is a complex clinical syndrome occurring as the end result of many different forms of heart disease. There are many different definitions and classifications of heart failure (Table 1.1) but a simple, practical definition of the syndrome of heart failure is that it is characterized by typical symptoms such as shortness of breath, exercise limitation and fatigue and clinical signs of peripheral and/or pulmonary congestion, associated with abnormalities of cardiac structure and function1. The syndrome of heart failure results in significant impairment of quality of life, more so than with many other chronic diseases2, and is associated with high morbidity and mortality. Heart failure frequently occurs in the setting of preserved left ventricular (LV) ejection fraction3,4 and thus a practical clinical definition of the syndrome1, rather than reliance on a single factor such as impaired LV ejection fraction, allows identification of the broad group of patients affected by this condition
